The role is responsible for supporting the development and implementation of an effective tissue viability and wound care management service. The role involves implementing a contemporary, evidence-based model of care focused on the prevention and management of skin and tissue injuries. The role will work collaboratively with multidisciplinary teams to ensure the highest standards of care and patient outcomes. This position requires a deep understanding of wound care principles, clinical expertise, and a commitment to continuous improvement in patient care practices. Responsibilities:
Lead wound care expertise by acting as the primary resource and liaison for staff on tissue viability and skin integrity management.
Deliver evidence-based care through thorough assessments of skin and tissue injuries, recommending tailored treatment plans to optimize patient outcomes.
Support quality improvement by contributing to audits, KPI data collection, and incidence/prevalence studies that drive continuous enhancement of care standards.
Collaborate across teams to develop and implement policies, procedures, and action plans that strengthen wound care practices and patient safety.
Educate and empower staff by providing training, competency evaluations, and ongoing support to ensure best practices in tissue viability are consistently applied.
Promote patient-centered care by conducting pre-admission wound assessments, facilitating referrals for complex cases, and ensuring timely follow-up of new referrals.
Engage patients and families by developing clear educational materials and offering guidance to support understanding and participation in wound care management. Qualifications:
