Roles & Responsibilities

Providing direct patient care in the home setting, including administering medication, monitoring vital signs, and assisting with activities of daily living.
Collaborating with physicians and other healthcare professionals to develop and implement individualized care plans.
Educating patients and their families on disease management, medication administration, and self-care techniques.
Assessing patients\' needs and creating comprehensive care plans based on their medical conditions and personal preferences.
Monitoring and evaluating patient outcomes to ensure the effectiveness of the care plan and making necessary adjustments as needed.
Documenting patient progress, interventions, and any changes in their condition to maintain accurate and up-to-date medical records.
Providing emotional support and counseling to patients and their families to help them cope with the challenges of their medical conditions.
Collaborating with other healthcare professionals and community resources to coordinate and facilitate the delivery of comprehensive care.
Adhering to all regulatory and ethical standards in the delivery of patient care, including maintaining patient confidentiality and privacy.
Continuously updating knowledge and skills through professional development opportunities and staying current on the latest advancements in home healthcare.
