The Case Manager will be responsible for providing comprehensive case management services to individuals or families in need. The Case Manager will assess client needs, develop and implement a service plan, and monitor progress toward goals. The Case Manager will work collaboratively with other service providers and community resources to ensure that clients receive appropriate services and support.
Responsibilities:
Conduct initial client assessments to identify needs and develop a service plan
Provide ongoing case management services, including regular check-ins and follow-up
Monitor progress toward goals and adjust the service plan as needed
Collaborate with other service providers and community resources to coordinate services and support
Advocate for clients and connect them with appropriate resources and services
Maintain accurate and up-to-date case files and documentation
Provide crisis intervention and support as needed
Participate in team meetings and case conferences
Keep up-to-date with industry standards and best practices for case management
Requirements:
Bachelor\'s degree in social work, psychology, or related field
4-5+ years of experience in case management or related field
Knowledge of social services and community resources
Strong interpersonal and communication skills
Ability to work independently and as part of a team
Excellent organizational and time management skills
Strong problem-solving and critical-thinking skills
Ability to maintain confidentiality and professional boundaries
Experience working with diverse populations and cultural sensitivity
Valid driver\'s license and reliable transportation
