Job Summary: - Clinical Case Manager (Patient Acquisition) is an integral part of the business development team and serves as the first impression for new patient prospects who are seeking an initial consultation with one of our physicians. - Clinical Case Manager (Patient Acquisition) plays an essential role in new patient acquisition for this active medical practice. Provides an prospect to represent the practice brand, learn about medical conditions and procedures, and collaborate with the medical team. - Responsible for evaluating patients\' treatment plans, monitoring patient well-being, creating case progress reports, and ensuring patients are well informed regarding their care and treatment options. - Excellent knowledge of case management principles and the ability to act as an advocate for patients in need. : - Handles case assignments by admitting patients to the medical facility, reviewing records, and conducting orientation. - Coordinates and provides patient care based on an approved treatment plan. - Facilitates aspects of the treatment plan, such as care coordination and discussions with the family of the patient. - Records all case information, such as care procedures, appointments, and check-ups. - Arranges the intervention of social services, health, or governmental agencies when needed. - Determines the need for therapeutic, psycho-social, or psychiatric evaluations based on patient progress. - Interacts with patients to ensure they are informed about their treatment plan and answers any questions they may have. - Evaluates patients\' progress and writing case reports to determine case closure. - Builds strong relationships with the medical team throughout the case management process. Job Requirements: - Bachelor\'s degree in nursing, or a relevant healthcare field. - A healthcare-related license, certification, or registration is preferred. - A minimum of 3 years\xe2\x80\x99 experience as a Clinical Case Manager, or a similar healthcare role within the markets of Abu Dhabi and Al Ain. - Extensive knowledge of case management principles and healthcare management. - Ability to work with patients with challenging behaviours. - Good communication and interpersonal skills. - Ability to be compassionate, empathetic, patient, and calm when working with patients.
