Overview

Reporting to Mubadala Health Senior Director of Clinical Development, the Clinical Case Manager (CCM) roles is assessment, planning, communication, education, resource care coordination, collaboration, and service facilitation following the Standards of Practice as per Case Management Society of America (CMSA). They are based on the needs and values of the patient and are accomplished in collaboration with all service providers as described by CMSA. This role will assist in the development of cross referrals across MH, referrals of patients from the medical community within UAE, and a focus on growing MH market share of patients.

Responsibilities

Job Summary
  • Responsible for ensuring effective clinical coordination across the continuum of care and implementation of evidence-based care pathways and use of clinic time for all physicians and health care professionals.
  • Facilitates process improvements or development projects to achieve desired outcomes both in terms of clinical and business outcomes
  • Solicits input from internal and external stakeholders, including persons served, referral sources, clinical staff, payers, and others in the broader community regarding needs and then incorporates these into further program development as appropriate.
  • Monitoring the clinical referrals network and ensuring compliance with established processes and systems and resolves issues that can jeopardize clinical development initiatives implementation.
  • Acts as a liaison between the Clinical Development Department and MH assets.
  • Conducts audits and data collection as instructed by the Senior Director, Clinical Development, or designee.
Leadership
  • Models\xe2\x80\x99 organization\xe2\x80\x99s core values and promotes translation of these values into work practices. Ensure quality standards are maintained.
  • Promotes and maintains working relationships among nursing personnel, medical staff, other hospital employees, patients and the public.
  • Develops conflict-resolution skills to mitigate division among colleagues at all levels to foster teamwork and to provide a positive patient environment by role modelling positive behavior to the staff.
  • Provides clinical and administrative support to all units / departments in the hospital after Office hours.
  • Provides a positive patient environment/customer friendly service by role modeling positive behavior to the staff.

  • Demonstrates excellence in leadership ability.

  • Conducts audits and data collection as instructed by the Senior Director, Clinical Development, or designee.
  • Performs any other duties as directed by Senior Director, Clinical Development.
Patient Flow Management
  • Conducts frequent rounds in all clinical departments to ensure customer satisfaction, resolve service/quality issues with follow through.
  • Coordinates and communicates with all relevant healthcare staff to ensure patient flow is smooth. Escalates appropriately if any delays are noted within the system.
  • Leads the development, implementation, and continual improvement of patient flow
  • Optimizes patient safety and experience
Education
  • Provide peer support to other team and facilitate others junior staff competence and specialist skill acquisition.
  • Show evidence of up-to-date evidence-based practice and customer service expertise amongst the team to enhance quality of service and staff development.
  • Presents/delivers training within appropriate subjects and where appropriate presents at conferences nationally or internationally where required.
  • Supports an environment that allows staff to acquire skills, identify and develop new ideas that ensures commitment to the organizational goals.
Performance Im[provement/Risk Management
  • Ensures compliance of unit/department hospital policies and procedures.
  • Promotes patients\xe2\x80\x99 clinical safety and risk management practices amongst clinical staff.
  • Ensures that patient care facilities, equipment and supplies meet required standards for safe patient care provision and ensure that staff understand Occupational Health and Safety legislation which relates to a safe environment.
Qualifications

Education
  • Bachelor\xe2\x80\x99s Degree from an Accredited School of Nursing/University
Experience
  • A minimum of 5 -7 years post graduate experience
  • Minimum 3 years within people management
  • Minimum of 3 years\xe2\x80\x99 experience within a health care environment with experience
