Job Description

Job Responsibilities:

1. Patient Care Coordination

Assess patient eligibility and suitability for home health care services.

Develop individualized care plans based on physician's orders and patient needs.

Coordinate and schedule nursing visits, physiotherapy, and other home care services.

Monitor and evaluate patient progress, adjusting care plans as needed.

2. Clinical Oversight

Supervise and guide home care nurses to ensure quality and continuity of care.

Ensure compliance with clinical protocols and HAAD/DOH regulations.

Perform periodic home visits to assess nursing performance and patient satisfaction.

Facilitate interdisciplinary case discussions and clinical review meetings.

3. Staff Management & Training

Assist in recruiting, onboarding, and training of home care staff.

Monitor nurse performance and provide coaching, mentoring, and performance evaluations.

Ensure staff are updated with clinical competencies, infection control practices, and documentation standards.

4. Administrative Duties

Maintain accurate and timely documentation of patient records and nurse reports.

Liaise with physicians, insurance providers, and family members for care coordination.

Ensure timely submission of reports to the Department of Health - Abu Dhabi (DOH).

Assist in audits, quality improvement initiatives, and policy updates.

5. Compliance & Quality Assurance

Ensure compliance with UAE healthcare regulations and HAAD/DOH licensing requirements.

Implement infection prevention and control measures in home care settings.

Participate in incident reporting, risk management, and follow-up on corrective actions.

6. Communication & Family Support

Act as the main point of contact for patients and families regarding care plans and updates.

Educate patients and caregivers on treatment plans, medications, and home care techniques.

Handle patient complaints or concerns professionally and escalate when needed.

Pay: From AED5,000.00 per month
