Job Summary

We are seeking compassionate and dedicated healthcare professionals to join our

Home Care team

. The

Home Care Nurse

will provide skilled nursing care to patients in their homes, ensuring comfort, safety, and adherence to medical plans. The

Nurse Assistant

will support patients with daily living activities and assist the nurse in delivering quality care.

Responsibilities

For Home Care Nurse:

Conduct patient assessments, including health history and physical exams. Develop and implement personalized care plans in collaboration with physicians. Administer medications, IVs, and wound care as prescribed. Monitor vital signs and report changes to the healthcare team. Educate patients and families on disease management and home care procedures. Maintain accurate medical records and ensure compliance with safety protocols. Coordinate with other healthcare professionals for holistic patient care.

For Nurse Assistant:

Assist patients with daily activities (bathing, grooming, feeding). Provide companionship and emotional support. Help maintain a clean and safe environment. Monitor patient comfort and promptly report concerns to the nurse. Support mobility and exercise routines as directed. Follow infection control and safety guidelines.

Qualifications:

Home Care Nurse:

Valid RN license (Preferably DHA/DOH) Minimum 2 year of nursing experience (home care preferred). Strong communication and organizational skills. Ability to work independently and travel to patient homes.

Nurse Assistant:

Certified Nursing Assistant (CNA) or equivalent. Previous experience in home care or healthcare setting preferred. Compassionate and patient-focused attitude.
