Job Description

We are seeking qualified and experienced Home Care Nurse and aides to provide professional medical assistance for an elderly patient at home.

Patient Care Needs Include:

Skilled Nursing Service:

Wound Care

Medication administration (Oral, insulin, Injections)

Daily Suctioning (oral / tracheostomy-up to 2x/day)

Chronic Condition Monitoring (e.g. Diabetes, Hypertension)

Post Surgical recovery support

Home Health Aide Support:

Assistance with personal Hygiene (Bathing, grooming, dressing)

Mobility and transferring support

Light housekeeping related to patient care

Therapy Services (if applicable):

Physical Therapy- Mobility, Strength and balance exercises.

Occupational Therapy- Assistance with daily living activities

Speech Therapy-Support with speech and swallowing

Care Coordination:

Communication with Doctors and family

Help with scheduling appointments and accessing resources

Post Hospitalization/ Transitional Care:

Follow-up support and medication management after discharge

Respite Care:

Temporary Coverage to Support the primary caregiver

Important Notes:

Suctioning Details: Daily Oral/tracheostomy suctioning (equipment provided)

Caregiver Requirements: Must be trained in suctioning procedures, infections control and basic emergency responses.

Emergency Protocols: Please explain your approach to handling respiratory distress or equipment malfunctions.

Exclusions: No IV therapy, Hospice or palliative care required.
