Reporting to the Service Line Manager, the Diabetes Educator role is responsible for providing comprehensive clinical education and care to diabetic patients including diabetes management, administering medications, blood glucose monitoring techniques and dietary/nutritional requirements to improve the patient\xe2\x80\x99s lifestyle conditions and ensure effective treatment in line with MH and DOH/DHA policies, procedures, guidelines and standards.

Organizational Culture & Compliance
Contributes to the development and improvement of \'business unit, division, department, section\' policies and procedures while ensuring compliance with the applicable standards and requirements of Joint Commission International (JCI), Legislation, Quality Standards, DOH/DHA, and all other applicable regulatory bodies.
Participates in meetings and maintains professionalism and confidentiality as per MH\xe2\x80\x99s standard code of conduct.
Diabetic Education
Identifies and assesses the patient\xe2\x80\x99s needs by obtaining and verifying patient information, anthropometric and biochemical data as well as any clinical, medical and surgical history.
Develops, implements and maintains individualized treatment plans based on the patients\xe2\x80\x99 assessment and evaluation of factors that may affect the outcome of the treatment and promotes successful self-management.
Provides patients and their family members with clinical education related to the administration of oral medication and insulin doses, injection of medications, diet and nutritional requirements, prevention and treatment of complications as well as blood glucose monitoring and self-monitoring techniques.
Provides patients and their family members with advice on exercising behavioral changes during diabetic treatments to improve lifestyle conditions and achieve treatment goals that reduce risks and optimize health outcomes.
Arranges referrals to other medical caregivers if required and tracks changes in patient\xe2\x80\x99s condition and treatments/interventions.
Informs Physicians on the initiation and/or adjustments required in the administration of insulin and oral hypoglycemic agents to the patient\xe2\x80\x99s body.
Prioritizes caseload in accordance with primary healthcare principles and the available resources in order to ensure coverage of all cases.
Develops and maintains clinical documents and patient records ensuring their accuracy and timely completion and captures detailed information and services provided to the patient.
Qualifications
ESSENTIAL
Completion of specialty qualification as per the recognized specialty certificate (DOH/DHA guidelines)
EXPERIENCE
Minimum of 2 years of experience post qualification in the relevant field
CERTIFICATION AND LICENSURE
DOH/DHA Medical License
