Job Description

Checks and verifies medication reconciliation on admission and discharge. Performs Drug-Drug, Drug-Lab, Drug Food and Drug-Disease Monitoring and offers alternatives when applicable. Provides medication information to healthcare professionals such as Prescribers and Nurses through pharmacy newsletters and lectures. Participates in patient care round to assess the status of the patient's health problems and determines whether the prescribed medications are optimally meeting the patient's needs and goals of care. Participates in Clinical Pharmacy activities such as antimicrobial stewardship, drug protocol development and management for the facility. Documents all clinical activities and interventions accurately and completely, as per policy and procedure. Is responsible for publishing monthly pharmacy educational bulletin. Demonstrates proficiency in aseptic technique during the preparation of sterile pharmaceutical admixtures. Demonstrates working knowledge of departmental equipment and software where applicable for assigned duties. Communicates effectively with healthcare professionals, patients, and patient family members where appropriate. Provides in-service education to Medical, Nursing and Pharmacy personnel on medication intricacies as well as Medication management policy and procedure manual. Participates in the orientation and training of newly hired Pharmacists and Technicians. Serves as a speaker on various QCHP approved CPD activities organized in the facility. Maintains professional competency and skills enough to meet current and future needs of the facility. Adheres to policies and procedures relating to Medication Safety and works on strategies to minimize medication errors and patient harm. Monitors and reports medication errors in the facility. Provides patients with thorough medication counselling, including how to best take his or her medications. Develops instruction cards on all the drugs included in the hospital formulary. Supports and participates in the goals of the organization's quality improvement. Maintains and monitor all records of narcotics and psychotropic drugs transactions in the hospital as regulated by M.O.P.H., and to report to pharmacy head about any error related. Verifies medication orders for accuracy and clinical appropriateness, checks medications prepared for accuracy prior to dispensing and notifies appropriate persons if changes are required. Contributes to the achievement of goals of the department through personal effort and influence over others. Provides professional, courteous customer service to all customer types (patients/family, Nursing, Medical Staff, others). Actively collaborates and demonstrates teamwork with pharmacy teams to ensure drug distribution is communicated and delivered safely and effectively. Is responsible for monitoring and standardization of crash trolley medication as per the MOPH List. Is responsible for creating and review of Hospital Formulary for updating and deletion of products. Is also sharing in revising Medication Management policy and procedures if needed. Additional responsibilities as directed by the department head.
