The Case Management Officer assists patients, families, and significant others with their social, environmental, financial, and emotional problems that are related to illnesses or to receipt of care.
Responsibilities
Responsible for the immediate contact of families/patients to arrange visits to the facility and meetings with the relevant staff as directed by the
Director of Case Management (DCM) within the designated site and to coordinate Patient Acquisition follow up. * To arrange and visit with key stakeholder groups Case Managers, Social Workers, and other government agencies at least 3 times per week or as
directed by the DCM. * To facilitate the Patient Acquisition Teams relationships with Key Stakeholders.
Attend all Patient Acquisition meetings to ensure coordination and communication of all activities.
To participate in all Patient Acquisition activities within their designated area as coordinated and organized by the DCM.
To facilitate the communication with the referring organizations as delegated by the DCM in a timely manner.
Actively participates in the recruitment and retention of patients as part of the multi-disciplinary team by ongoing communication and follow up with
key family members and organizations. * Be available by phone from 8am to 8pm Monday-Friday or as needed.
Facilitates the best practice in line with the facilities discharge policy.
Participates in care planning and timeline decisions for discharge.
Acts as a resource for patients, their families, and the Multidisciplinary team Involves in planning, assessment and anticipates patients\xe2\x80\x99 and their
family needs before discharge. * Educate and ensure the patient and their families are fully involved in the discharge process.
Meet the multidisciplinary team in order to update and discuss the progress of the patient\xe2\x80\x99s discharge.
Ensure confidentiality and exercises discretion in all matters relating to the patient.
Become knowledgeable on the community support system.
Coordinate access to equipment supplies to meet discharge needs.
Work closely with the concerned clinical staff in identifying and providing timely quality care and service for residents.
Collaborates and liaises with the multidisciplinary team to promote smooth and efficient discharge for patients.
Attends daily rounds to enhance communication between multidisciplinary team meetings.
Provides orientation programs to all new Clinical personnel explaining the responsibilities, roles, and service offered during discharge of the
patient. Also, provide continuing education to staff related application of standards/crite
Qualifications
ESSENTIAL
Bachelor Degree
Prior experience in relevant field, such as, Customer Care, Public Relations, Social and Volunteer work.
