- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative long term nursing actions.
- Review and updates as needed the comprehensive assessment, clinical notes, Physician Summaries, Plan of Care and communicates patient status changes and updates to the primary physician regularly , receives their orders as required.
- Counsels the patient and family in meeting nursing and related needs.
- Provides clinical oversight and supervision of Health Care Assistants.
- Provides skilled care and education to the patient and family as appropriate per assessment and plan of care.
- Liaison with members of the multi-disciplinary team, to ensure optimal outcomes for the patient.
- Complies with all organizational policies.
- Skillful in basic and advance nursing procedures such as cardio-pulmonary resuscitation,medication administration, Nebulization, ECG,IV Cannulation, Medication administration, post op care ,caring of unconscious patient , sample collection ,RT insertions, feeding and care, Vaccinations, vitals monitoring , wound care and dressing, care of patient on dialysis, personal hygiene care(skin care, bath and oral care) , urinary catheter care, tracheostomy care, oxygen administration, ADL assistance, perineal care , and enema administration.
