Basic Function

Performs psychosocial counseling and social work activities using social work skills to help in planning, directing, evaluating and resolving the patient's social and psychological problems in all JHAH facilities (Dhahran and districts). Works in coordination with professional medical staff and patient's family, helping in problems related to their family and extended family relationship in the context of medical care and psycho-social problems. Assures adherence to established JHAH and JCIA (Joint Commission on International Accreditation) policies and standards in relation to providing social work services in a hospital setting.

Scope

Responsible to respond to the Consultation Referral for Social Work Service made out by the General Practice, Specialty and Sub-Specialty physicians and authorized medical staff. Provides counseling, performs home/social evaluation, financial evaluation, discharge planning, etc. for in-patients receiving medical treatment in JHAH facilities. Performs adequate coordination with the patients relatives, supervisors and other persons significant to the patient and his illness. Also responsible to document the outcome in Epic. Incumbent is responsible to provide the required actions, including the interviewing and psycho-social counseling of patients, their families and persona significant to the patient and facilitate adjustment of these individuals to the impact of illness and psycho-social problems. This is to give better medical outcome, increase the employee's productivity, social adaptations and to promote maximum benefits from the Saudi Aramco Health Care Services. Incumbent is also responsible for making known to physicians/concerned medical staff the social factors relating to illness. Makes adequate follow-ups for continuity of counseling and other social work activities. Adheres to Confidentiality of Patient Information. Incumbent participates in the ongoing activities, philosophy and objectives of the social work program in JHAH. Incumbent does not have any direct financial responsibility but significantly saves the company financial resources by way of active participation and collaboration in Discharge Planning of patients for potential home discharge cases, directing patients to available resources in and out of the Community. Receives supervision and guidance from the Senior Social Workers and Service Manager for appropriate handling of complicated cases in relevant field, as needed. Reports to the Manager, Social Work Services.

Principal Contacts

Direct contacts with all in-patients of varying lengths, depending on the case, with all eligible patients in the in-patient and out-patient setting. Direct contacts with the referring physicians, chiefs of different specialties and conceited medical staff. Direct contacts with the referred patients relatives, and/or persons of significant importance to the patient's illness. Periodic contacts with Personnel Department, Government Affairs Representatives, Industrial Security, Social Insurance and all levels of Johns Hopkins Aramco Healthcare (JHAH) personnel. Contacts with Senior Social Worker, Manager of Social Work Services to consult and receive guidance for the complicated cases.

Principal Duties

Responds to Consultations for Social Work intervention from relevant healthcare staff or case findings Interviews referred patients to establish and assess the nature and extent of their problems (complaints, nature, cause, components and impact). Makes psychosocial assessment and counseling of the referred patients and their relatives that have relationship with present medical problem. Performs social work supportive counseling, education and other assistance on the patient and their family members as the case requires. Discusses/consults with and/or informs concerned medical staff regarding psychosocial factors and other aspects to assure integrated plan of care. Documents all cases (initial and progress notes findings) in the patient EHR. Helps the patient to cope and adjust with various types of illness/psychological/social conditions by implementing psychosocial therapeutic support through individual and/or family counseling. Makes appropriate and interval follow-ups on the patients and their families as per their plan of care. Participates in the Daily/Weekly In-patient Unit rounds with other health care providers, Participate as directed by Manager, SWSU or his designee during internal and external disasters in the JHAH facilities. Attends Communications Meetings, in-service training,, discharge planning meetings, units Quality Improvement Program, etc. Prepares and submit reports covering assigned area of responsibility, such as, monthly reports, daily reports and assignment reports as requested.

Education

Bachelor degree in Social Work, Sociology or psychology from recognized university.

Experience

Minimum 3 years of experience in social work privation at medical setting demonstrating ability to perform social work assignments independently

Certifications/Other requirements as applicable

Excellent command of oral and written English. Arabic language is mandatory. Requires broad knowledge of the field of social work and understanding of the cultural and social aspects in dealing with multi-nationals.
