A home care nurse performs a variety of duties for care and comfort for patients requiring medical services. He/she must utilize specialized skills and judgment when delivering care to patients. Implementation and evaluation of patient care while performing nursing procedures to treat and rehabilitate patients.
1. MAIN RESPONSIBILITIES
1.1.
Assuring that the safety and physical comfort of the patient are meet as well as the patient's emotional and spiritual needs.
1.2.
Performing nursing procedures for which his or her training has provided the necessary skill and judgment.
1.3.
Regular checking of expiry date of medicine and consumables and keeping proper record.
1.4.
Having regular checking of medical equipment and noted in a checklist.
1.5.
Assisting with the rehabilitation of patient according to the care plan directed by the physician.
1.6.
Observing and recording all pertinent information such as nurse's notes and reporting to the appropriate supervisor and physician.
1.7.
Teaching patients, family members, and auxiliary nursing personnel proper health maintenance care.
1.8.
All treatment and diagnostic procedures are carried out under medical direction or proper prescription (written) from patients Primary Physician; whereas self medications or medications without prescription by their Primary Physician such as herbal, natural, etc. should have consultation and proper prescription upon administration to avoid interference with current intervention, treatment, and management.
1.9.
To provide clean and organize environment for patient that is suitable for his/her medical needs, (e.g. medicine cabinet, medical supplies, bed side equipment's)
1.10.
To inform members of the family of any changes in patients, condition, behavior.
1.11.
To be loyal and dedicated to his/her work, to be responsible and to show empathy toward his/her patient and show high level of professionalism in nursing performance and towards patient and family members
1.12.
To report unusual, unprofessional, uncivil ways or acts happened inside the patient home/setting.
1.13.
To observe, report, and document client responses to prescribed treatment and care.
1.14.
To communicate progress and concerns to HHC Manager.
1.15.
To prepare Monthly Nursing Care Plan.
1.16.
To prepare detailed Home Care Nursing Management Report as often as needed.
1. MINIMUM REQUIREMENTS
1.1.
Bachelor degree in nursing
1.2.
Current Valid Registered Nurse HAAD License
1.3.
Valid BLS Certificate/Identification Card
1.4.
ACLS, PALS relevant to the area of specialty
1.5.
Strong nursing assessment skills; well-developed communication skills.
1.6.
Able to convey technical information to patients in terms they can understand and apply.
1.7.
Strong organizational skills.
1.8.
Proficient in current nursing practices and procedures.
1.9.
2 years clinical experience post qualification inappropriate setting
