The Registered Nurse I role is responsible for providing and maintaining a high standard of individualized nursing care to patients and their family members according to current best-evidence based standards, using best judgement at all times in alignment with DOH/DHA and MH policies procedures, guidelines and standards.
Responsibilities
Handling inbound and outbound calls and provide advice based on best practice guidelines and business updates.
Telemedical triaging with community-based patients
Completing nursing assessments and triaging according to patient condition.
Reviewing patient medical records via Malaffi for validation of medical conditions
Using Medical Guidelines to handle and prioritize calls for the Nursing Hub Team
Using an electronic Patient Management System for management and documentation.
Active promotion of Telemedical skills such as being able to virtually assess patient\xe2\x80\x99s medical history, physical status and providing appropriate management.
Arranging for patients referrals to the appropriate level of care as indicated and according to the guidelines.
Technical support to the Physician team in accordance with the clinical processes
Supporting of non- medical team including (medical knowledge, triaging, technical support, patient registration and push the flow of work)
Responsible for planning, executing, and evaluating patient care for follow up cases.
Demonstrating the Telecare mission, purpose, values, and beliefs in multi-language. Provide physical, mental, emotional, and social health education to promote patient health and wellness.
Actively participating in multidisciplinary team meetings to plan for patient wellbeing and quality service.
Maintaining high level of patient experience and ensure patient safety in all interactions with patients.
Actively engaging in creating a Training Program for new recruits, including preparing teaching materials, and presenting information.
Qualifications
Education
Bachelor\xe2\x80\x99s Degree in Nursing OR Associate degree in Nursing from Canada or USA with minimum 2 years courseduration and successful completion of the National exam:
o USA: NCLEX \xe2\x80\x93 RN o Canada: Licensing Exam
Experience
Primary health care experience (general medicine, pediatrics, gynecology, general internal medicine)
Ambulatory care experience (ambulatory care)
Emergency Care experience (but not intensive care station)
Certification and Licensure
DOH/DHA Medical Licensure
