Job Description
The home care nurse\xe2\x80\x99s responsibilities include performing patient visits and monitoring their conditions, conducting tests, and writing notes on the daily basis:
- Conducts an individualized patient assessment, prioritizing the data collection based on the patient\xe2\x80\x99s immediate condition or needs within timeframe specified by client facility\xe2\x80\x99s protocols.
- Conducts ongoing assessments as determined by patient\xe2\x80\x99s condition and reprioritizes care accordingly.
- Develops plan of care that is individualized for the patient reflecting collaboration with the Head Nurse.
- Performs appropriate treatments as ordered by physician in an accurate and timely manner.
- Performs therapeutic nursing interventions as established by individualized plan of care for the patient and his/her family.
- Delivers care with appropriate age and cultural competence to specific patient according to individualized needs.
- Provides individualized patient/family education customized to the patient and his/her family.
- Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution.
- Maintains confidentiality in matters related to patient, family and client facility staff.
- Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient\xe2\x80\x99s and family\xe2\x80\x99s diversity, preserving their autonomy, dignity and rights.
- Reports patient condition to appropriate personnel during each shift.
- Bathing and/or assisted bathing for clients who are too ill to take bath in the bathroom or for bedridden or handicapped clients;
- Changing of clothes, undergarments, continence aids and any soiled sheets;
- Brushing of teeth and cleaning of dentures;
- Toileting and other elimination needs;
- Simple hair trimming;
- Lifting, transferring and positioning of clients;
- Assisting with oral and/or tube feeding;
- Assisting in light housekeeping and laundry if the client and/or his caregiver is unable to do so;
- Simple wound dressing;
- Simple errands, where necessary and appropriate;
- Medication reminders and monitoring of medication compliance;
- Administering medication;
- Administer medication and take and record vitals.
- Prepare meals according to instructions provided by parents.
- Feed infants on a predefined schedule and guide them to feed themselves when necessary.
- Change diapers and bathe infants.
- Introduce infants to activities and toys aimed at the physical and cognitive development.
- Manage infants\xe2\x80\x99 social, emotional, and physical needs.
- Rock and verbally comfort infants when they are agitated.
- Talk to infants to facilitate relationship building.
- Ensure that all toys provided are clean and safe for infants to use.
- Perform emergency procedures such as First Aid and CPR in the event of an emergency.
- Perform laundry and ironing services of infants\xe2\x80\x99 clothes
- Ensure that there is a constant supply of diapers and other baby products
- Perform grocery shopping activities aimed at buying the right food for infants
- Build a curriculum based on the differing needs of infants (while working in a daycare setting)
- Manage positive interaction activities targeted at providing stimulus, growth, and development.
- Maintain daily records and reports for infants.
- Communicate with parents or guardians regarding infants\xe2\x80\x99 progress or problems.
- Prepare various teaching materials in accordance with infants\xe2\x80\x99 ages (while working in a daycare setting)
- Administer prescribed medication if the need arises.
- Observe and report any significant behavioral patterns to the parents.
