The Registered Nurse (Home Care) is responsible for providing high-quality nursing care to patients in their homes, ensuring safety, dignity, and adherence to clinical protocols and ethical standards. The role requires delivering personalized care, monitoring health status, and coordinating with the healthcare team.
Key Responsibilities:
- Provide direct patient care based on individualized care plans.
- Administer medications and treatments as prescribed.
- Monitor vital signs and report any changes in the patient's condition.
- Assist patients with activities of daily living (ADLs).
- Educate patients and families on care plans, medications, and health maintenance.
- Document all care provided accurately and timely.
- Collaborate with physicians, therapists, and other healthcare professionals.
- Maintain infection control and patient safety standards.
- Respond to medical emergencies appropriately.
- Ensure all equipment and supplies are maintained and used correctly.
Requirements:
- Valid DOH (HAAD) or DHA license or eligibility.
- Bachelor's Degree in Nursing.
- Good communication and interpersonal skills.
- Ability to work independently and adapt to different home environments.
