Job Title:

Referral Officer - Home Rehabilitation Service

The Referral Officer will be responsible for coordinating, managing, and facilitating referrals for home rehabilitation services. This role ensures smooth communication between hospitals, clinics, patients, families, and the homecare team to secure appropriate referrals and provide timely support to patients requiring rehabilitation care at home.

Key Responsibilities

Build and maintain strong relationships with hospitals, clinics, physicians, and other referral sources. Receive and process patient referrals for home rehabilitation services. Verify referral documents and ensure all required information is complete and accurate. Coordinate with the clinical and operations team to assess service eligibility and patient needs. Provide patients and families with information about home rehabilitation services. Track, monitor, and report on referral activities and conversion rates. Maintain an updated database of referral sources and patient records. Support awareness and promotional activities to strengthen referral networks. Ensure compliance with organizational policies and healthcare regulatory requirements.

Qualifications & Requirements

Bachelor's degree in healthcare, business administration, or related field (preferred). Experience in healthcare referral coordination, patient services, or medical administration. Strong communication and interpersonal skills. Knowledge of homecare/rehabilitation services is an advantage. Organizational and multitasking abilities. Proficiency in MS Office and patient management systems.
