Job Description

Dispense medications accurately and safely to patients based on prescriptions provided by healthcare professionals, ensuring adherence to dosage instructions, drug interactions, and patient allergies. Verify the accuracy and appropriateness of medication orders.

Review and interpret prescriptions, ensuring the selection of the most suitable medications and dosages for individual patients. Collaborate with physicians and other healthcare providers to address any medication-related concerns or queries.

Maintain a comprehensive and updated inventory of pharmaceuticals, including ordering, receiving, and storing medications in compliance with regulatory requirements and safety protocols. Monitor medication stock levels and expiration dates.

Provide medication counseling and education to patients and their families, explaining medication usage, potential side effects, and precautions. Offer guidance on proper medication administration, storage, and adherence to treatment plans.

Collaborate with healthcare teams to develop and implement medication management protocols, including medication reconciliation, drug utilization review, and adverse drug event monitoring. Contribute to medication-related policies and procedures.

Perform medication reviews and therapeutic drug monitoring to optimize patient outcomes and minimize medication-related risks. Assess the appropriateness and effectiveness of drug therapies, and make recommendations for therapy adjustments as necessary.

Utilize computerized systems and software to document medication-related information accurately and maintain patient profiles. Ensure compliance with legal and regulatory requirements regarding patient confidentiality and data protection.

Participate in medication safety initiatives, such as medication error reporting, root cause analysis, and implementation of preventive measures. Stay updated with medication safety guidelines and contribute to a culture of continuous improvement in patient safety.

Stay informed about new drugs, dosage forms, and pharmaceutical products, as well as changes in treatment guidelines and drug regulations. Participate in continuing education activities to enhance knowledge and skills in pharmacy practice.

Collaborate with other healthcare professionals, including physicians, nurses, and technicians, to provide interdisciplinary patient care. Act as a resource person for medication-related information and contribute to the development of evidence-based treatment protocols.
