Reporting to the Senior Therapist/ Manager, Rehabilitation, the Occupational Therapist is responsible to demonstrate clinical knowledge and understanding that is relevant to their area of practice and underpins their individual scope of practice. The role must be able to recognise their role and responsibilities within the patients plan of care and demonstrate knowledge and understanding of anatomy and physiology, evidence-based practice, and Occupational Therapy practice of service users through the life span. The Occupational Therapist must also operate in line with M42 and DOH/DHA policies, procedures, guidelines, and standards.
Responsibilities
Clinical and Operational Responsibilities
Interprets referrals and conducts initial evaluation, problem identification and diagnosis of physiological, psychological, and cognitive functioning.
Develops, delivers, and modifies treatment plans and provides appropriate treatment for an individual\xe2\x80\x99s physiological, cognitive and/or psychological needs within the context of the person as an individual.
Defines patient-centred goals and educates the patient, caregivers, and interdisciplinary team of these.
Informs patients about their recommended Occupational Therapy treatment plans and treatment modalities, and ensures understanding, consent, and cooperation from the patient prior to commencing the therapy.
Uses relevant standardized outcome measures to evaluate progress, appropriately monitors patient tolerance and response to therapeutic interventions and provides progressions and modifications to the treatment plan as necessary to achieve the established patient centred goals.
Conducts constant risk assessments and adheres to necessary safety measures during therapy sessions.
Uses appropriate strategies to effectively facilitate client self-management and health promotion education to enhance health and well-being. * Develops and maintains records and reports of all patient assessments and treatments in an accurate and timely manner that reflect specific information and services provided to the patient.
Ensures that administrative tasks linked to patient care, including documentation, billing, insurance submissions, and requests are completed as per policy and regulations.
Respects the skills, expertise, and contributions of colleagues, advocating for the patient and referring on when appropriate. Liaises with other health care providers as necessary to ensure appropriate and timely interventions for the patient.
Collaborates with the interdisciplinary team to provide individualized care from point of the first contact throughout the episode of care, incorporating the continuum of care and rehabilitation goals.
Proficient analysis skills enabling high quality and highly specialized individual evidence-based treatment plans.
Identifies & manages deterioration with patient conditions, initiates appropriate action, and escalates concerns appropriately.
Demonstrates service excellence and advocacy to always support the patient rights and dignity. * Actively participates in meetings regarding patient care for own caseload and where additional support has been identified/ prioritized or requested by the Line Manager.
Provides comprehensive clinical handover and reports as required to ensure patient safety and continuity of high quality of care.
Participates in the maintenance of sufficient inventory of material supplies and equipment for performance of duties.
Participate in marketing and patient access activities as directed and where their clinical competence will enhance these activities.
Qualifications
Education
Bachelor\xe2\x80\x99s degree in Occupational Therapy OR Direct entry of a recognized program with attainment of a MSc degree in Occupational Therapy
Experience
Minimum of 6 years as an Occupational Therapist in a large hospital with relevant experience with patients with the same diagnosis seen in acute care hospital or rehabilitation facilities.
Teaching and mentoring skills and experience.
