OverviewReporting to the Lead, Medical Transcriptionist, the Medical Transcriptionist is responsible for providing assistance to Physicians and relevant medical caregivers in related administrative activities including but not limited to transcribing patients\xe2\x80\x99 medical reports and dictated notes of Physicians to be stored in the patients\xe2\x80\x99 electronic medical records (EMR) as well as reviewing, editing and developing relevant documentation if and when required.ResponsibilitiesMedical Transcription
  • Transcribes patients\xe2\x80\x99 medical reports and any notes of Physicians and/or relevant medical caregivers through dictation systems; written and/or live dictation for permanent storage of patient data in the electronic medical
record (EMR). * Responsible for implementation of retention and destruction policies and procedures for all medical record documentation as necessary.
  • Develops and sends insurance request forms to be sent to the relevant Physician for approval, verifies the insurance approvals for the prescribed treatments if applicable and assists physicians to respond to rejections in a timely manner.
  • Reviews and documents patient visits and treatments as well as telephone orders in an accurate and timely manner to ensure regular update of the required documents on relevant systems, in line with the asset regulations and policies.
  • Provides and retrieves patient records by conducting activities including but not limited to processing, abstracting, analyzing and coding to enhance provision of transparent and high-quality patient care.
  • Assists the Physicians and relevant clinical caregivers in administrative activities including but not limited to external referral letters, sick leave requests for patients and escort leaves.
  • Liaises with relevant caregivers in processing requests for protected medical information and the release of patient information in adherence to the asset protocols and regulations.
  • Records all requests and relevant documentation in the patient\'s medical record as per the disposition of request, type of access, date and name of person processing the request.
  • Assists patients in accurately completing the appropriate forms and documents required for storage in medical records and ease of consultation.
  • Ensure patient visit and treatments on the system are accurate with billing requirements and provided services.
Daily Roles and Responsibilities
  • Comply with accrediting (JCIA/DOH/DHA) and other monitoring bodies and agencies
  • Assist in the transfer and migration of patient data from paper to electronic format. The process and phases of transferring data will comply with governing regulations, facility policies requirement and accreditation as
per the Medical Records /Management of Communication and Information. * Support with transcription of dictation (using dictation systems or live dictation or paper based) for the medical record, implementing DOH/DHA standard best practices for the transcription of medical reports for permanent storage in the patient\xe2\x80\x99s electronic medical record (EMR).
  • Support the physicians and the healthcare team with administrative related to documentation; sick leave, external referrals and escort leave.
  • Support with preparation and send referral/ insurance request forms for their assigned physicians if applicable.
  • Liaise with the ROI officer of the HIM team for processing requests for protected medical information and release of patient information in accordance with facility policies and procedures.
  • Assist with determining the financial status of patients and their eligibility for health services; assist patients in accurately completing appropriate forms and documents for the required information
Qualifications
  • Vocational/Technical Education
  • Up to 3 years of progressive career experience
  • Ability to transcribe physician notes and
encode in EMR systems. * Excellent computer, typing skills, verbal communication and interpersonal skills.
  • Understanding of medical terminologies
  • Proficient in related medical record digital
  • systems
  • Excellent analytical, problem-solving and time management skills
  • Fluency in written and spoken English
  • Excellent grammar, spelling and punctuation skills
