This involves monitoring vital signs, evaluating symptoms, and assessing the patient\'s overall well-being. Medication Management: Administering medications as prescribed by the physician, ensuring proper dosage and timing. They also educate patients and their families about medication usage and potential side effects. Wound Care: Providing specialised care for wounds, including cleaning, dressing changes, and monitoring for signs of infection. They may also teach patients and caregivers how to manage wound care between visits. Personal Care: Assisting patients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility. They may also help with feeding and monitoring dietary intake. Patient Education: Educating patients and their families about their health conditions, treatment plans, and self-care techniques. This includes teaching them how to manage chronic conditions, prevent complications, and promote overall wellness. Monitoring and Reporting: Regularly monitoring the patient\'s health status and reporting any changes or concerns to the healthcare team. They maintain detailed records of patient assessments, interventions, and progress.
