Responsibilities: Conduct assessments and create care plans for patients in their homes Administer medications and perform treatments as prescribed by the patient\xe2\x80\x99s physician Monitor and document patient vital signs, symptoms, and reactions to medications or treatments Educate patients and their families on their medical conditions, medications, and treatments Coordinate care with other healthcare providers, including physicians, therapists, and social workers Communicate regularly with the patient\xe2\x80\x99s physician to ensure continuity of care Maintain accurate patient records and documentation Provide emotional support to patients and their families Requirements: Registered Nurse (RN) licensure Experience in home health or a related field is preferred Excellent communication and interpersonal skills Ability to work independently and make sound clinical judgments Flexibility and adaptability to changing patient needs and schedules Basic computer skills for documentation and communication Valid driver\xe2\x80\x99s license and reliable transportation Overall, a home care nurse plays an important role in the healthcare system by providing personalized care to patients in their own homes.
