Key Responsibilities:
Provide direct nursing care including wound care, medication administration, IV therapy, and chronic disease management.
Conduct initial and ongoing assessments of patient health and home environment.
Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
Educate patients and families on treatments, medications, and health maintenance.
Document all visits and care provided accurately using electronic health records (EHR).
Monitor and respond to changes in patient conditions and communicate concerns promptly.
Follow all policies, procedures, and safety standards in accordance with state and agency regulations.
Qualifications:
Active and unrestricted RN or LPN license in [Insert State].
