Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans for elderly patients
- Provide ongoing medical care and treatment for chronic conditions commonly found in the geriatric population
- Collaborate with a multidisciplinary team to coordinate and manage the overall health and well-being of patients
- Educate patients and their families on disease prevention, management, and end-of-life care
- Perform diagnostic tests, order and interpret medical tests, and prescribe medications as needed
- Monitor and evaluate changes in patients' conditions and adjust treatment plans accordingly
- Stay updated with the latest research and advancements in geriatric medicine to provide evidence-based care
- Serve as a resource and consultant for other healthcare professionals in the organization
Requirements:
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution
- Board certification in Geriatric Medicine (ABIM or AOBIM)
- Current and unrestricted medical license in the state of practice
- Minimum of 2 years of experience working with elderly patients in a clinical setting
- Excellent communication and interpersonal skills
- Ability to handle complex medical cases with sensitivity and empathy
- Strong analytical and problem-solving abilities
- Familiarity with electronic health record systems and medical software
