Job Description

1.1. Coordinates home care nursing with other members of the healthcare team. Updates care plan as needed and as patient\xe2\x80\x99s condition changes. 1.2. Documents the patient\xe2\x80\x98s condition accurately and concisely, using correct charting format: in electronic medical record, notifies the physician of condition changes. 1.3. Performs nursing duties within clinical scope of practice and professional code of conduct as directed by Nursing Administration, DOH PQR and UAE Nursing Norms 1.4. Teaches, supervises and counsels the patient and family members regarding the nursing care needs and other related problems. 1.5. Demonstrate high level of infection prevention knowledge and actions in the Homecare setting and reports any issue according to PHS policy and infection framework. 1.6. Works within the policy of homecare to deliver the mission, vision and core values of the organization. 1.7. Practices nursing within the legal limits of Health Authority Abu Dhabi, understands, and anticipates legal consequences of nursing action. 1.8. Maintains and updates current knowledge in nursing procedures. 1.9. Ensures privacy for the patient and confidentiality of condition and personal information. 1.10. Participates in care conferences, staff meetings and committees. 1.11. Cooperates and acts as liaison with the referring facility. 1.12. Participates with other Staff Nurses in taking weekend, holiday and night calls on a rotation basis as scheduled. 1.13. Participates in the planning, operation and over-all evaluation of the homecare service. 1.14. Provides appropriate documentation activities and interventions according to facility 1.15. standards. Assesses plans and provides nursing care to newborn, infant, children, adolescent, adult and geriatric patients. 1.1. Assesses and evaluates patient\xe2\x80\x99s physical and psychosocial needs upon admission to the homecare facility. 1.2. Identifies patient\xe2\x80\x99s clinical problems, symptoms and behavioral changes. 1.3. Develops a nursing care plan that integrates the medical plan of care, nursing diagnosis, realistic goals, nursing interventions and established discharge criteria. 1.4. Carries out physician\xe2\x80\x99s orders in accordance with the facility policies. 1.5. Initiate\xe2\x80\x99s preventive and rehabilitative nursing procedures as appropriate for the patients care and safety, without infringing on the patient\xe2\x80\x99s rights according to Health Authority Abu Dhabi scope and standards of nursing practice of homecare nurses including but not limited to: 1.5.1. Assessment of patient\xe2\x80\x99s vital signs 1.5.2. Monitoring of patient\xe2\x80\x99s blood glucose levels and providing teaching and support to the family 1.5.3. Preparation and administration of medications in accordance with the 8 Rights of Medication Administration and as prescribed by the physician. 1.5.4. Assessment for Oxygen Therapy requirements Tube Feeding Management 1.5.5. Nasopharyngeal and tracheostomy care 1.5.6. Straight Catheterization 1.5.7. Pain Management via non-pharmacological strategies Wound care management 1.5.8. Ostomy care and maintenance Basic Life Support. 1.6. Promotes patient\xe2\x80\x99s independence by establishing patient care goals; teaching and counseling patient and family and reinforcing their understanding of diseases, medications, and self-care skills. 1.7. Identifies patient service requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand service requirements. 1.8. Coordinates with the tending physician/higher center staff in managing emergency medical situation that needs patient\xe2\x80\x99s to be referred to higher center. 1.9. Assesses patient and family about the disease process in accordance with their degree of acceptance. 1.10. Educates the patient and family about the disease process in accordance with their degree of understanding. 1.11. Educates, Supervises and counsels the patient and family members regarding the nursing 1.12. care needs and other related problems.
